How to fill in and submit the statistical eForm
Search eForm
After successfully logging in and accessing the Electronic Data Collection (eCollection), in the Statistical eForms tab, the Search Electronic eForm window will appear with the Year, Survey eForm and Reporting Unit items.
You can search for the eForm by filling in the individual items from the drop-down menu:
- Under Year, select the year for which you are required to provide data.
- Under eForms, you select the eForm you are required to complete.
- Under Reporting unit, the identification number and the name of the company will be filled in automatically.
- Then click on View electronic forms.
- A line with the identification number, Period, Status items and other data will be displayed. Position the mouse cursor over your identification number in the line with the current period and and click to view the eForm.
Status of the survey
Before the survey is available in the system to the reporting units, it is in the status Initialised. This status does not allow you to fill in the data into the eForm.
Only when the Statistical Office of the Slovak Republic switches the status of the survey to status In progress will the eForm for filling in the data become available.
You will be informed of the availability of electronic data collection (status of survay is in progress) by sending a Notification of Invitation to fulfil the reporting obligation to your e-mail address. If you do not have a completed e-mail address in our system, the Notice will be delivered to your electronic inbox.
Contact Information in the eForm
Contact information on the Department of the Statistical Office of the Slovak Republic
In the introductory part of the eForm, under the respondent's address, the information which relevant regional department processes your report is given.
Contact information of the processor questionnaire for methodological assistance
At the beginning of the data section of the eForm, in module 100387, the name, telephone number and e-mail of the staff member who will provide you with methodological assistance and answer your questions are given.
Contact information for the person completing the eForm
Module 100307 is intended for filling in the contact information of the person who completes the eForm on behalf of the reporting unit with the obligation to submit the statistical return. These data should be completed in the first period of inclusion in the statistical survey. For any change in the contact informations, they need to be updated in the following periods.

You move to the next survey variable (data field).
Inner and outer bar on the right side of the eForm
Use the inner bar to navigate through the eForm modules. The outer one takes you to the beginning and end of the eForm.
List of modules in the eForm
Click on the appropriate module number to be set to the beginning of the module in the form. You can also use the Page Up and Page Down buttons to move between modules.
Question mark icon - methodological help
Click on the Question Mark icon in the modules of the form to display the methodological help.
Survey variables – Code list
Some survey variables have a drop-down menu (arrow) to which the Code list is linked. You select a value from it. When working in the eForm, you can turn this function off.
Error message and red box
Appears if an incorrect value type is entered (e.g. if the data is to be numeric, letters cannot be entered; some data cannot have a negative value; entering a dot instead of a decimal point; the number of digits in the numeric data, and the number of decimal places, cannot be higher than the defined range, etc.).
Asterisk *
The data is mandatory. A red colour indicates that the mandatory data is not filled in. When typing or correcting the value of a survey variable which is of type date you will select from the available calendar.
Key functionalities of the eForm
There are functionality buttons at the bottom and top of the eForm:
Save as work in progress
The system performs predefined checks on the format of the completed survey variables and saves the report as a work in progress.
Check eForm
The predefined checks of the filled data are run, any errors detected are displayed by the system under the eForm in an error message.
Printing eForm
It is only possible to print the completed eForm after it has been sent to the system. The Methodological explanatory notes on the content of the eForm are printed with each eForm.
Printing eForm without explanatory notes
Allows you to print the complete eForm with the uploaded data without the Methodological Explanatory Notes.
Printing error messages
Error messages to be displayed after checking the eForm.
Back without saving
Changes you have made to the form will not be saved.
Initialization status
Initiating the state will clear all the data you have filled in and the form will be updated to its original version.
New comment
A comment is a tool for you to communicate with the statement processor.
Send to
To submit the report, press the Submit button. All predefined controls over the report will run before the form is submitted. The system will display an error message if it identifies an error.
Error list
Green marking indicates an informative error
It is used for preventive checking of filled or not filled data. If the data you have filled in is correct, consider this message to be informative, e.g. number format not followed, number of digits exceeded, mandatory item not filled in. You can also submit the eForm with informative errors.
Orange indicates a serious error
This is an error such as an unfilled item or module, e.g. an invalid entry in a dial-bound discovery variable, an incorrect checksum. You will be able to submit the eForm even with these errors, but you will be contacted by the Statistical Office of the Slovak Republic and asked to correct them.
A red mark indicates a blocking error that prevents the eForm from being submitted
This is an error such as incorrect data format, e.g. a text string instead of a number. Once the errors have been corrected, click the Submit button. If the system continues to display errors, click Cancel. Remove the errors again and resubmit the completed eForm.
Checking the report and generating errors before sending
You can also check the report with completed data on an ad-hoc way. When you press the Check Report button, the system runs checks on the eForm and generates a table with a summary of the errors in the eForm.
Confirmation of successful submission of the eForm
After the eForm has been sent to the system, a window will appear on the screen to indicate that the submission was successful. At the same time, you will receive an automatic notification message of submission preferably to your email address entered in Edit Custom Profile.
